RN Case Manager

StartDate: 2/9/2026 Pay Rate: $1800.00 - $2700.00

POSITION SUMMARY – RN Case Manager

POSITION DUTIES –

• Care Coordination / Continuum of Care / Discharge Planning / Patient Focus and Care Plans / Resource Utilization
• Patient assessment
• Referrals to HHC, SNF. A-Rehab, SNF, LTAC, LTC
• Arrange DME
• Arrange IV antibiotics if needed
• Arrange acute to acute transfers
• Arrange transportation on discharge
• Participate on daily MDR
  
  

MINIMUM REQUIRED QUALIFICATIONS – FL/compact RN license; BLS; 2 years recent acute hospital case management experience; Cerner & Midas experience

PREFERRED QUALIFICATIONS – Have all InterQual courses taken and already passed the test before starting

LENGTH OF ASSIGNMENT – 13 weeks

SHIFT / HOURS PER WEEK – 8A-4:30P

SYSTEMS – Cerner / Midas

START DATE – ASAP
